All 7th grade students are required to receive the TDaP and Meningococcal vaccine and must submit a record with the school by August 30th in order to continue to attend school. If you choose to not vaccinate your child, you will need to submit opt out paperwork. If you have questions, please contact the school nurse at 937-865-0011.
