Please print and complete all of the above forms. To register your student, you will also need to provide copies of the following documents:

           Birth certificate 
           Immunization records
           Social Security card (or provide number)
           Proof of residency (utility bill, water bill, etc.)
           Certified copy of custody papers or divorce decree
